Every family is required to volunteer at least 5 times per semester as a teacher's aide. If you do not wish to volunteer, you can also pay an opt-out fee. Please select either the volunteer background check option or the opt out fee. If you are registering multiple students and one of the classes is full, please send an email to tsamuels@commonwealthacademyky.org to see if we have any overflow space. Liability Waiver (All): In consideration of my student(s)'s participation in Commonwealth Academy, I hereby release, waive, and hold harmless Restoration House Family Worship Center, Commonwealth Academy, its organizers, participating parents, volunteers, instructors, and any representatives (collectively referred to as “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained by me or my student(s), whether caused by negligence or otherwise, during participation in Commonwealth Academy events.
Wildroots Participants Only: Please list ALL students, siblings, and parents that will participate in Wildroots:
Wildroots Liability Waiver (Wildroots participants only): Wildroots conducts outdoor nature classes, including without limitation, outdoor play and hiking. Playing outdoors and/or hiking can be inherently dangerous activities. While participating in Wildroots activities, my child(ren) and/or myself may be exposed to dangers and hazards, including but not limited to: falling rocks, inclement weather, slippery walkways or terrain, uncertain trail conditions, trip hazards, hostile or aggressive wildlife, falls, skin cuts or scrapes, sprains, bone fractures, concussions, plant allergens or other irritants, biting or stinging insects, negligence of others, temporary or permanent injuries, and/or death. I and/or my child(ren) listed above have voluntarily chosen to participate in these Activities. I and/or my child(ren) listed above, voluntarily, knowingly, and expressly assume the above mentioned risks as well as other risks not listed that are part of, related to, or arising from participation in the Activity.
Medical Release (All): In the event of a medical emergency involving my student(s), I authorize the Commonwealth Academy leaders or designated adult volunteers to seek emergency medical care, including transportation, diagnosis, and treatment. I understand that reasonable efforts will be made to contact me prior to such treatment. I agree to assume all financial responsibility for any medical care provided. Commonwealth Academy will not assume any responsibility for health care due to injury or incident.
Photo and Media Release (All): I, the parent or legal guardian of the student(s) listed above, grant permission for Commonwealth Academy to photograph or video my child(ren) during co-op activities and to use those images for official purposes, including the Academy’s Facebook page, website, printed materials, or other electronic media.
Policy Agreement (All): I hereby acknowledge that I am the parent or legal guardian of the minor student(s) listed above and voluntarily agree to the Commonwealth Academy Policies for participation in Commonwealth Academy.
