2026 Future 15 Nomination Form

NOMINATION CRITERIA:

Young Professional (YP) must live and/or work in the Fox Cities*
YP must be currently employed
YP should be active in community service
YP must be 40 years of age or younger at the time of the awards
Applications sent to each nominee must be completed by the nominated person and then submitted for consideration (only one nomination is necessary; multiple nominations do not strengthen candidate’s chance at winning)
If selected, the recipient must agree to be featured in a video shown the night of the event and also on social media
Previous winners are not eligible to reapply

*The Fox Cities Chamber serves all of the Fox Cities communities within the counties of Outagamie, Calumet and the northern portion of Winnebago.
