  • This survey is to understand what it’s like to care for residents in our nursing homes, including what gets in the way of delivering the level of care you want to provide and that residents deserve.

    The information you share will be used in a public report on nursing homes. It’s important for the public to hear from you, as the person doing the work on the floor every day, what staffing, access to supplies, food quality, and ownership or management changes are really like.

    The survey is anonymous. If you are open to follow-up, you will have the option to leave us your name at the end of the survey.
