    The Agreements and Required Forms section of the preschool enrollment form contains important documents that require your acknowledgment and signature. These documents include the terms and conditions of enrollment, waivers, membership agreements, and other forms necessary to provide the best possible care for your child.

    Please read each section carefully and sign where indicated. Your signature indicates that you have read, understand, and agree to the terms and conditions set forth in each document.

    It is important to note that failure to sign any of the required documents may delay your child's enrollment or the inability to provide care for your child. We appreciate your cooperation in completing these necessary documents and helping us to ensure a safe and positive experience for your child.

    • Terms and Conditions (required) 
      1. I hereby enroll my child in the Rosen JCC Learning Center. I understand and agree to the terms as stated on this form, and I hereby agree to pay the full amount due for those enrollment terms as per the 2026-27 Registration & Tuition Fees sheet. This DOES NOT APPLY to VPK-only programs. I understand that the registration fee, if applicable for the selected program, will be charged upon receipt of this form using my payment method on file. If I do not have a payment method currently on file, I will be contacted to complete payment. All Rosen JCC Learning Center fees, including tuition and registration, are non-refundable.

      2. I authorize my bank to honor EFTs for the payment of Rosen JCC fees, including but not limited to, tuition and registrations. EFTs for monthly tuition are typically withdrawn on the 25th of the month. I understand that I will be assessed a fee for any returned payments, and authorize my credit card on file to be used as a backup method of payment for any unpaid fees.

      3. I understand that a JCC family membership is required for participation in the Rosen JCC Learning Center program, with the exception of the VPK-only program. Non-payment of membership fees may result in termination of services. If I am not currently a member, I will be contacted to establish my membership prior to enrollment.

      4. I understand that this agreement signifies a financial commitment for the 2026-27 school year. This DOES NOT APPLY to VPK-only programs.
      5. I understand that the school year for the Rosen JCC Learning Center programs runs from August 11, 2026, to July 31, 2027, during which the school is closed for various holidays throughout the year. However, please note exceptions for specific programs: The Pre-K program operates from August 11, 2026, to May 29, 2027, and the VPK-only program runs from August 11, 2026, to May 29, 2027. These dates reflect the unique schedules for each of these programs, distinguishing them from the standard academic calendar.

      6. I understand that families with an outstanding balance for 30 days or more may be subject to withdrawal of their child/children from program unless alternative arrangements have been made with the accounting department. This DOES NOT APPLY to VPK-only programs.

      7. If your child is enrolled in a program that concludes at 12:30 PM, please ensure they are collected promptly. A late fee of $15 will be charged if your child is not picked up by 12:35 PM. To accommodate potential delays in the pickup car line for children enrolled in programs ending at 3:00 PM, we provide an extended grace period until 3:30 PM for their dismissal. After this time, a late pickup fee of $15 will be applied. For those enrolled until 6:00 PM, please note that a late pickup fee of $15 will also be incurred for any student not promptly picked up by 6:05 PM. Furthermore, pickups after 6:15 PM will result in a significant late fee of $100. It's important to understand that repeated instances of late collection beyond these grace periods may lead to expulsion from the program. This policy is designed to ensure our staff can efficiently fulfill their end-of-day responsibilities while maintaining the highest quality of care for all children.

      8. I agree to read the Rosen JCC Learning Center Parent Handbook, available on our website, and will abide by all Rosen JCC and the Rosen JCC Learning Center policies.

      9. I understand that it is my responsibility to download and register for the Brightwheel app. Brightwheel is used to receive important communications from teachers and staff at the Rosen JCC. 

      10. I understand that my child’s enrollment in the Rosen JCC Learning Center will not be complete until all physical and immunization forms are turned in. If a child does not have these forms turned in to the Rosen JCC Learning Center prior to their first day of school, or they expire during the school year, they will not be able to attend until they are updated. Please check the expiration date of all documents, as it is the parent’s responsibility to keep these forms updated. There is no exception to this policy.

      11. I grant permission to the Rosen JCC Learning Center to assess my child's development and request further evaluation if deemed necessary in the sole discretion of the school's director. If it is determined that a child requires additional staff resources to participate in our program, it will be the parents' financial responsibility to cover required services. If a child is diagnosed with autism or a related disorder, they will be required to attend school with a therapist or paraprofessional during their daily schedule unless otherwise determined by the school's director.

      12. I agree to sign and return all required forms by the necessary deadline. Additional forms in our Parent Packet will be made available prior to the start of the school year. Throughout the year, you will be sent forms that need to be electronically signed. These must be signed and returned immediately. There is no exception to this policy.

      13. I hereby give permission to the Rosen JCC Learning Center at the Rosen JCC to take photographs, videos, and audio recordings of my child to send via Brightwheel. I understand that the media taken may include my child's image, likeness, and work completed during their time at the Rosen JCC Learning Center. I acknowledge that the media taken may be used by the Rosen JCC for promotional purposes, including but not limited to, print and digital media, social media, and the center's website. I acknowledge that if I do not agree to this policy, I must notify the Rosen JCC in writing. I hereby release the Rosen JCC and its employees from any and all claims and demands arising out of or in connection with the use of the photographs, videos, and audio recordings, including without limitation any and all claims for invasion of privacy, defamation, or financial compensation. By signing below, I acknowledge that I have read and fully understand this agreement and am voluntarily giving my consent to the use of photographs, videos, and audio recordings of my child as described above.

      14. I give my child(ren) permission to participate in the Rosen JCC Learning Center activities. I understand that even when every reasonable precaution is taken, accidents can sometimes happen.

      15. I release the Rosen JCC and its staff, volunteers, members and officers from all liability for any injury, loss, or damage connected in any way whatsoever to me or my children’s participation in the JCC activities, whether on or off JCC’s premises. I authorize the JCC staff to act on their best judgment in the event of an emergency; including transporting myself or my child(ren) to the hospital and authorizing treatment by a physician.

      16. I understand and agree that tuition and fees cannot be refunded due to illness, vacation, health, safety, or weather-related closures. I understand that thirty (30) days written notice is required for program withdrawal or a fee of one month's tuition will be charged. I understand that if I withdraw with an unpaid balance, the Rosen JCC Learning Center will be authorized to charge my payment method on file for the balance due.

      17. I hereby give permission to childcare personnel at the Rosen JCC to access my child's files and records.

      18. The Rosen JCC Learning Center admits students of any race, gender, color, religion, national or ethnic origin and does not discriminate on the basis of any of these categories or any category protected by law in the administration of our educational policies, admission policies, financial aid, athletic or other programs.

      19. Expulsion Policy: A positive and constructive working relationship between the school and parent is essential to the fulfillment of the school’s educational purpose. Thus, the school reserves the right not to extend the privilege of enrollment or re-enrollment to a student if the school reasonably concludes that the actions of the parent make such a positive and constructive relationship impossible or otherwise seriously interferes with the school’s accomplishment of its educational purpose. Moreover, the school reserves the right to expel any student at any time if, in the judgment of the Rosen JCC Early Childhood Director, conduct of anyone directly associated with the student, including but not limited to the student’s parent, in or out of the school, is not keeping with the school’s accepted standards and principles. There will be no refund of current monthly tuition where such enforced withdrawal occurs, and any unpaid balances are payable in full according to the individual agreement. If, for any reason, it is in the best interest of the school, the school also reserves the right to withdraw an offer of enrollment or re-enrollment at any time, and to nullify an executive enrollment agreement.
    • By signing below, I agree that I have read and understand the terms of ECLC enrollment, the Membership Agreement and the Waiver, Release and Indemnification Agreement.

    • Membership Agreement (required) 
      1. I understand that memberships are paid in full on the 1st of the month and payments are not refundable.

      2. I understand that memberships are non-transferable.

      3. I understand monthly memberships fee will incur a $2 credit card service fee.

      4. I understand that all information must be correct and current.

      5. I understand as a member I am required to have FOB to have access into the facility.

      6. I understand that Month-To-Month Memberships are continuous and will remain in effect 30 days after cancellation.

      7. I understand membership fee subject to change. All Membership applications will remain as pending until reviewed and approved by management. Picture or Government ID are REQUIRED with application.

      8. I understand that I am responsible for any applicable registration fees, proration fees, 1st month, monthly dues and 30-day cancellation period dues.

      9. I understand that under Rosen JCC's Declined Credit Card/Returned Check Policy, I am responsible and AUTHORIZE a $25 fee to be charged to the debit/credit card listed on the Membership Payment Authorization Form for each credit card, EFT, and check draft that gets declined and for any check that gets returned.
    • By signing below, I agree that I have read and understand the terms of the Membership Agreement.

    • Membership Cancellation Policy (required) 
    • I understand that all membership cancellation requests must be submitted in writing to AlydaG@rosenjcc.org. I understand that I am responsible for all drafts including the 30-day cancellation period. I AUTHORIZE the Rosen JCC to charge my credit/debit card for any outstanding payments including Rosen JCC's Declined Credit Card/Returned Check fees within the 30-day cancellation period.

    • By signing below, I agree that I have read and understand the terms of the Membership Cancellation Policy.

    • Waiver, Release, And Indemnification Agreement (required) 
    • ln consideration for being permitted to utilize the facilities, services, and programs of the Rosen JCC (JCC) for any purpose, including but not limited to observation or use of facilities or equipment, or participation in any program affiliated with the JCC, without respect to location, the undersigned, for himself or herself and any personal representatives, heirs, and next of kin, hereby acknowledges, agrees and represents that he or she has, or immediately upon entering or participating will inspect and carefully consider such premises and facilities or the affiliated program. Lt is further warranted that such entry into the JCC for observation or use of any facilities or equipment or participation in such affiliated program constitutes an acknowledgment that such premises and all facilities and equipment thereon and such affiliated programs have been inspected and carefully considered and that the undersigned finds and accepts same as being safe and reasonably suited for the purpose of such observation, use, or participation.

      IN FURTHER CONSIDERATION OF BEING PERMITTED TO ENTER THE JCC FOR ANY PURPOSE, INCLUDING BUT NOT LIMITED TO OBSERVATION OR USE OF FACILITIES OR EQUIPMENT OR PARTICIPATION IN ANY PROGRAM AFFILIATED WITH THE JCC, WITHOUT RESPECT TO LOCATION, THE UNDERSIGNED HEREBY AGREES TO THE FOLLOWING:

      1. THE UNDERSIGNED HEREBY RELEASES, WAIVES, DISCHARGES, AND COVENANTS NOT TO SUE the JCC, its directors, officers, employees, and agents (hereinafter referred to as "releasees") from all liability to the undersigned, his personal representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands therefore on account of injury to the person or property resulting in death of the undersigned, whether caused by the negligence of the releasees or otherwise while the undersigned is in, upon, or about the premises or any facilities or equipment therein, or participating in any program affiliated with the JCC, without respect to location.

      2. THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage, or cost they may incur due to the presence of the undersigned in, upon, or about the JCC premises or in any way observing or using any facilities or equipment of the JCC or participating in any program affiliated with the JCC whether caused by the negligence of the releasees or otherwise.

      3. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY DEATH, OR PROPERTY DAMAGE due to negligence of releasees or otherwise while in, about, or upon the premises of the JCC and /or while using the premises or any facilities or equipment thereon or participating in any program affiliated with the JCC.

      THE UNDERSIGNED further expressly agrees that the foregoing RELEASE, WAIVER, AND INDEMNITY AGREEMENT is intended to be as broad and inclusive as is permitted by the law of the state of Florida and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect.

    • By signing below, I agree that I have read and understand the terms of the Waiver, Release, And Indemnification Agreement.

    • Acknowledgment, Release, And Waiver Of Liability Agreement For Loss, Injury, Or Illness (including COVID-19) [required] 
    • WHEREAS, the Jack & Lee Rosen Jewish Community Center ("Rosen JCC") is the operator of a community center facility located at 11184 S Apopka Vineland Road, Orlando, Florida, including without limitation the drive aisles, parking lots, sidewalks, basketball courts, fitness center, athletic fields and all exterior and interior portions of the Rosen JCC facilities (collectively the “Premises”), and is willing to permit the individuals signing this Agreement and their minor child(ren) to enter onto and into the Premises to use the common areas, facilities and amenities for all intended uses (the "Activities"), upon the terms and conditions of this Acknowledgement, Release and Waiver of Liability Agreement (the "Agreement");

      WHEREAS, the undersigned member, visitor or parent/guardian acknowledges, affirms, represents and warrants that neither the member, visitor nor the child(ren) identified below has experienced symptoms of COVID-19, tested positive for COVID-19 or has been exposed to any individual who has tested positive for COVID-19 in the last 14 days; and

      WHEREAS, to reduce the spread of COVID-19 to the members, visitors, guests, children and staff of the Premises, for good and valuable consideration, including but not limited to being provided access to and use of the Premises and participation in the Activities, the undersigned, on behalf of themselves and any minor children named below, hereby stipulate and agree that the above recitations are true and accurate, and further state, stipulate and agree to the below terms:

      1. Use of Premises. I understand and agree that I or my child may only use the Premises for the purpose of engaging in the Activities and must advise the Rosen JCC of any change of circumstances in the health of any of us or of any exposure by any of us to someone who is exhibiting symptoms or has tested positive for COVID-19 until further notice. I also agree to review and comply with all written memoranda and posted signage regarding safety practices and restrictions of use.

      2. Assumption of Risk. I understand and acknowledge that the Activities that take place on the Premises may be dangerous and may involve the risk that I or my child will contract a serious illness (including COVID-19) or injury, temporary or permanent disability, death, and/or property damage. I understand that both my presence on Premises and that the Activities that take place on the Premises may not be supervised and that the Rosen JCC does not provide medical services. I further acknowledge that any illness or injury sustained while on the Premises may be compounded by negligent or delayed medical service. I further acknowledge that Coronavirus, COVID-19, is an extremely contagious virus that spreads easily through person-to-person contact. Federal and state authorities recommend social distancing as a means to prevent the spread of the virus. COVID-19 can lead to sever illness, personal injury, permanent disability, and death. Participating in Rosen JCC Activities, programs or accessing the Premises and/or Rosen JCC facilities could increase the risk of contracting COVID-19. The Rosen JCC in no way warrants that COVID-19 infection will not occur through participation in Activities, Rosen JCC programs or through accessing the Premises.

      I VOLUNTARILY AND FREELY ASSUME ALL RISKS AND DANGERS THAT MAY OCCUR PURSUANT TO MY OR MY CHILD(REN)’S USE OF PREMISES AND PARTICIPATION IN THE ACTIVITIES ON THE PREMISES, INCLUDING THE RISK OF ILLNESS, INJURY, DEATH, OR PROPERTY DAMAGE.

      3. Release from Liability. I hereby agree, on behalf of myself, my child(ren), my heirs and my personal representatives or their heirs or personal representatives, to fully and forever discharge and release the Rosen JCC, their affiliates, and their respective partners, agents, operators, managers, employees, and representatives ("Released Parties") from any and all claims I or my child(ren) may have or hereinafter have for any illness, injury, temporary or permanent disability, death, damages, liabilities, expenses and/or causes of action, now known or hereinafter known in any jurisdiction in the world, attributable or relating in any manner to my entry upon and use of the Premises, whether caused by the negligence of the Rosen JCC or any of the Released Parties or by any other reason. I acknowledge and agree that this Release and Waiver of Liability is intended to be, and is, a complete release of any responsibility of the Released Parties for any and all illness (including COVID-19 or other communicable disease or illness), personal injuries, temporary or permanent disability, death, and/or property damage sustained by me while participating in the Activities on or using the Premises.

      4. Covenant Not to Sue. I agree, for myself, my child(ren) and any heirs, not to sue the Released Parties or initiate or assist in the prosecution of any claim for damages or cause of action against the Released Parties which I, my child or any heirs may have as a result of any illness, personal injury, death or property damage I or my child may sustain while participating in the Activities on or using the Premises.

      5. Responsibility for Personal Property. I acknowledge and agree that I am fully and solely responsible for any of my property and personal belongings that I bring onto the Premises and that the Rosen JCC will not be responsible for or provide any security for my property and personal belongings.   

      6. No Representations by the Rosen JCC. I acknowledge that the Rosen JCC makes no representation as to the condition of the Premises or the safety of any structures or equipment (including sanitation and disinfection) that may be used at the Premises. I accept and shall use the Premises in its "AS IS" condition. I acknowledge and agree that I am not relying upon any representation or statement by the Rosen JCC or the Rosen JCC's employees, agents, or representatives regarding this Agreement or the Premises, except to the extent such representations are expressly set forth in this Agreement, and any statements not expressly set forth within this Agreement shall have no force and effect and cannot form the basis for any cause of action later asserted.

      7. Governing Law and Venue. This Agreement will be governed by and interpreted in accordance with the laws of the State of Florida, without giving effect to the principles of conflicts of law of such state. I agree that any action arising out of this Agreement must be brought exclusively in any state or federal court located in Orange County, Florida.

      8. Waiver. No waiver of any term or right in this Agreement shall be effective unless in writing and signed by an authorized representative of the waiving party. The failure of any party to enforce any provision of this Agreement shall not be construed as a waiver or modification of such provision, or impairment of its right to enforce such provision or any other provision of this Agreement thereafter.

      9. Survival and Severability. Any provision of this Agreement providing for performance by either party after termination of this Agreement shall survive such termination and shall continue to be effective and enforceable. If any provision or portion of this Agreement shall be held by a court of competent jurisdiction to be illegal, invalid, or unenforceable, the remaining provisions or portions shall remain in full force and effect.

      10. Ratification; Entire Agreement; Modification; Binding Effect. Except as herein provided, all other terms and conditions set forth in any agreement between the parties remains in full force and effect as written. In the event of a direct conflict between this Agreement and any other agreement between the parties, the terms of this Agreement control. This Agreement is the entire agreement between the parties with respect to the subject matter of this Agreement and supersedes any prior agreement or communications between the parties, whether written, oral, electronic, or otherwise with respect to subject matter of this Agreement. No change, modification, amendment, or addition of or to this Agreement shall be valid unless in writing and signed by authorized representatives of the parties. This Agreement shall be binding upon and inure to the benefit of the successors, assigns, and legal representatives of the parties.

      11. Award of Attorneys' Fees and Costs. In the event of any dispute, litigation, or enforcement action(s) arising out of or related to this Agreement or any obligations set forth herein, the prevailing party shall be entitled to recover its reasonable attorneys' fees and costs through all appeals and including attorneys' fees related to litigating costs and attorneys' fees.

      12. Failure to Comply. The Rosen JCC shall have the absolute right to prohibit any individual (or minor child if applicable) who fails to comply with this Agreement from participating in the Activities and/or request that they leave the Premises.

      13. Notice to Minor Child's Natural Guardian

      NOTICE TO THE MINOR CHILD'S NATURAL GUARDIAN (as required by Florida Statute Section 744.301(3):

      READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITIES. YOU ARE AGREEING THAT, EVEN IF THE ROSEN JCC USES REASONABLE CARE IN PROVIDING THE ACTIVITIES, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THE ACTIVITIES BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM, YOU ARE GIVING UP YOUR CHILD's RIGHT AND YOUR RIGHT TO RECOVER FROM THE ROSEN JCC IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITIES.       14. Jury Trial Waiver. I HEREBY WAIVE, FOR MYSELF, MY CHILD(REN) AND ANY HEIRS, ANY RIGHT TO A TRIAL BY JURY IN ANY LITIGATION ARISING OUT OF, UNDER, OR IN CONNECTION WITH THIS AGREEMENT.

    • I hereby acknowledge that I have fully read and understand each of the above provisions. I acknowledge that I, personally, or my child/children (if applicable) is/are not experiencing symptoms of COVID-19 and that I, personally, or my child/children (if applicable) has/have not been exposed to anyone who has tested positive for COVID-19 within the last 14 days. I also acknowledge that I, as parent or guardian, have no symptoms of COVID-19 and have not been not been exposed to anyone who has tested positive for COVID-19 for the last 14 days. I am at least eighteen (18) years of age and fully competent, and I execute this agreement voluntarily and for adequate consideration intending to be fully bound.

  • Teacher Appreciation Fund

    Families are encouraged to participate in our Teacher Appreciation Fund, which is used to provide holiday gifts, gift cards, meals, and events in appreciation of our teachers.

    By selecting "Yes" below, you acknowledge that you will be charged $100 per child to contribute to the Teacher Appreciation Fund. Payments can be made from either a checking account that is on file, by check, or cash. You will be charged the first month your child attends school, and then two months later ($50 each payment).
