Warning: By signing this document, you acknowledge the risks of participating in Miso events and activities, and waive certain legal rights, including the right to sue, to the fullest extent permitted by law. Please read carefully.

Risk Acknowledgement & Waiver

By signing this form, I acknowledge that participating in Miso Volleyball tournaments and related activities involves inherent risks, including physical injury, illness, property damage, and other unforeseen loss or harm.

I confirm that I am voluntarily participating, that I am physically able to do so safely, and that I will follow all tournament rules, venue rules, and instructions from Miso Volleyball staff, referees, and officials.

In exchange for being allowed to participate, I release Miso Volleyball and its organizers, staff, volunteers, referees, sponsors, venue partners, and representatives from any claims or liabilities, including but not limited to personal injury, loss, damage, costs, death, or liability arising from or related to my attendance or participation, including claims arising from negligence, to the fullest extent permitted by law.

I understand that Miso Volleyball may remove me from an event, without refund, for safety reasons, rule violations, misconduct, or failure to follow directions from staff or officials.

I confirm that I am 18 years of age or older on the date of the event and that all information I provide in this form is true and accurate. Miso Volleyball reserves the right to refuse participation or remove any participant, without refund, if they are under 18 or if any information provided is false or misleading.

If I am injured or require medical attention during the event, I authorize Miso Volleyball to obtain first aid or emergency medical assistance if needed. I understand that I am responsible for any related costs.

I understand that photos and videos may be taken during Miso Volleyball events. I consent to Miso Volleyball using this content on its website, Instagram, and other promotional or social media channels. If I prefer not to be featured, I will notify Miso Volleyball in advance or at the event. While complete exclusion cannot be guaranteed, reasonable efforts will be made to respect such requests.
