Registration Disclaimer
By submitting this registration form, I acknowledge and agree to the following on behalf of the participating school/team:
1. Assumption of Risk & Liability Waiver
I understand that participation in athletic activities involves inherent risks, including but not limited to injury, illness, or property damage. The league, its organizers, staff, officials, and volunteers are not liable for any injuries or damages incurred during participation. It is the responsibility of the school/team to ensure that all student-athletes are physically cleared by a licensed medical professional to participate.
2. Medical Clearance
All student-athletes that participate have received proper medical clearance and are deemed fit to participate in athletic activities for the duration of the season.
3. Coach Certification
All coaches associated with this team are currently certified in CPR and AED, first aid, and concussion training, and will maintain up-to-date certification throughout the season.
4. Payment Obligation
By registering, the school confirms its financial commitment to pay all required league fees, regardless of future withdrawal or schedule changes.
5. Rules & Handbook Acknowledgment
Our school/teams have access, reviewed, and understand the league’s rules and handbook. Coaches, players, and administrators agree to abide by all league policies, procedures, and codes of conduct.
6. Onboarding
Your school will be responsible for submitting coach contact information, team availability, and a school roster forms when requested. Schools must meet the deadlines for these forms to ensure accurate scheduling and smooth season planning.
Failure to comply with any of the above requirements may result in disqualification from participation, suspension, or other disciplinary action as determined by the league administration.
