1. Assumption of Risk
I, the undersigned parent/legal guardian, understand that participation in soccer and related activities involves inherent risks, including but not limited to: falls, collisions, contact with other players, equipment, or field conditions, and the risk of serious injury, illness, or death.
I voluntarily assume all risks associated with my child’s participation in Inter BK Soccer Academy activities, including practices, games, camps, clinics, travel, and related events.
2. Release of Liability
I hereby release, waive, discharge, and hold harmless Inter BK Soccer Academy, including its coaches, trainers, staff, volunteers, directors, and board members, from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss sustained by my child during participation.
3. Medical Responsibility
I understand and agree that I am solely responsible for any and all medical expenses incurred as a result of my child’s participation.
If insured: I will use my own insurance coverage.
If uninsured: I accept full financial responsibility for all medical costs.
4. Medical Authorization
In the event of an emergency, I authorize Inter BK Soccer Academy staff, coaches, or representatives to obtain emergency medical treatment for my child, including transportation to a medical facility.
I understand that every reasonable effort will be made to contact me prior to treatment. I consent to treatment to save my child life or incase a ambulance need to call or EMT for treatment.
5. Health Disclosure
I confirm that my child is physically able to participate in soccer activities and has no medical conditions that would prevent safe participation, unless disclosed on this form.
6. Photo & Media Release
I grant permission for Inter BK Soccer Academy to use photographs and/or video of my child for promotional purposes, including social media, website, and marketing materials.
7. Code of Conduct Acknowledgment
I agree that my child and family will follow all rules, policies, and codes of conduct set by Inter BK Soccer Academy. Failure to comply may result in removal without refund.
8. Acknowledgment & Agreement
I have read this waiver carefully and fully understand its contents. I understand that I am giving up certain legal rights by signing this document, and I voluntarily agree to its terms.
