  • In accordance with Whitefield's Corporate Credit Card Policy, cardholders have a responsibility to retain support for all charges by providing a dated, itemized receipt that identifies the item(s) purchased.

    If a receipt is lost and cannot be recovered, complete this form. Employee and supervisor signatures are required before uploading it to the Truist Enterprise Spend Platform in lieu of the receipt.
