CONSENT TO PARTICIPATE / PHOTO RELEASE
I give my child/ward permission to participate in The O League’s Programs. Furthermore, on behalf of my child, and myself I release any claim of liability arising from my child’s participation in any activity related to the program. Additionally, I waive responsibility of the staff, volunteers, sponsors, affiliates and facilities used in the event of injury to my child. I give permission to The O League to use photographs and videos/audios of my child obtained while participating in the program for marketing opportunities to provide information and publicity for the program. In case of emergency and a parent cannot be reached and immediate medical assistance is necessary, my child may be taken to the nearest hospital and treated by the emergency room physician. I give my permission for any attending physician and medical service personnel to tender medical treatment to my child.*
