Fannan Scholarship Awardee Form

For those of you receiving multi-year scholarships. We will need you to submit this form every year so we can verify your proof of enrollment. Due to the high volume of scholarships that we process, we suggest that you email melissa@marcellafoundation.org after you submit this form to confirm our receipt and see if we have any questions.

Check with your college on how they would like us to issue the check. For example: UC Regents, UCLA, University of California Los Angeles. They should tell you how they receive checks written to their financial institution.

Ask your college where they would like to receive a scholarship check. Don't assume it is the financial aid office. You need to call them to ensure we are sending it to the right place.

The file could be a screenshot or a picture listing your enrolled classes. The document needs to include the school, your name, and student ID number.

When do they need this payment?

Please allow up to 2-3 weeks for us to process your scholarship award.

What if other scholarships or financial aid cover my tuition?
If your tuition is fully covered by other scholarships or financial aid for all four years, please reach out to us. We may be able to provide a direct disbursement to help support housing and other personal expenses.
