Cheyenne County Community Center ADULT LEAGUE TEAM CAPTAIN – RELEASE OF LIABILITY & ASSUMPTION OF RISK For Athletic Leagues, Games, Practices, and Related Activities
As the designated Team Captain for the adult league team listed below, and in consideration of my team being allowed to participate in athletic leagues, competitions, practices, tournaments, scrimmages, training sessions, and any related activities organized by or associated with the Cheyenne County Community Center (CCCC), whether at the Community Center or at any off-site gym, school, field, park, or facility used for scheduled games or events, I, _______________________, acknowledge, represent, and agree that: I am authorized to sign on behalf of all players and participants on my team’s roster, and I acknowledge that participation in athletic activities involves inherent risks, including but not limited to slips, falls, collisions, equipment failure, contact with other participants, and other hazards that may result in injury, illness, permanent disability, or death. While rules, conditioning, equipment, and personal responsibility may reduce these risks, serious injury can still occur; and, I knowingly and freely assume all such risks on behalf of myself and all rostered participants, both known and unknown, even if arising from the negligence of the Releasees or others, and assume full responsibility for participation by the team I represent; and, I agree that all team participants will follow all rules, policies, facility expectations, and safety instructions provided by Cheyenne County Community Center staff, officials, volunteers, and facility personnel at any location. If any unsafe condition, hazard, or concern is observed, I will ensure affected participants discontinue play and notify CCCC staff immediately; and, I, on behalf of myself, my team participants, and all heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS the Cheyenne County Community Center, its directors, officers, employees, volunteers, officials, coaches, event staff, other participants, sponsoring agencies, sponsors, advertisers, and any owners or operators of facilities or game sites used for athletic events (“Releasees”) from any and all claims of injury, disability, death, or loss or damage to person or property arising out of participation or presence at any athletic competition or activity, whether caused by the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
