I, the undersigned participant, agree to voluntarily participate in activities provided by Twisted Lotus LLC, DBA Twisted Lotus Jiu Jitsu & Yoga, including but not limited to Brazilian Jiu Jitsu, yoga, self-defense, breath work and movement-based wellness, in various locations, including mobile settings.

I understand that these activities involve inherent risks, including physical injury, and I assume all risks, known or unknown. I affirm that I am physically able to participate and will inform the instructors of any medical conditions.

I hereby release, waive, and discharge Twisted Lotus LLC, DBA Twisted Lotus Jiu Jitsu & Yoga, its owners (including Josh Beno), instructors, contractors, partners, and affiliates, from any and all liability, claims, or demands arising from my participation, including negligence. I agree to indemnify and hold the Company harmless from any claims arising out of my actions.

I grant permission for the Company to use any photographs or videos taken during classes for promotional purposes, unless I opt out in writing. I have read and understood this waiver and agree to its terms voluntarily. This agreement is governed by the laws of the State of Arizona.
