This form will be opened until 30 Jul 2026, 11.59 p.m.
1. This form is intended exclusively for members who wish to convert a Standard Order, processed using account credits, into a Loyalty Rewards Order for the current month
2. To be eligible for this adjustment, members must meet the following requirements:
  - Be actively enrolled in the Loyalty Rewards Program.
  - Ensure the Standard Order has a minimum value of 100 PV in order to qualify for conversion to a Loyalty Rewards Order. Standard Orders below 100 PV will not be eligible for adjustment.
  - Ensure payment for the Standard Order was made using account credits to qualify for the adjustment.
3. Please note that by submitting this request and providing your consent for the adjustment, you acknowledge and agree that any excess shipping fee paid under the Standard Order, compared to the Loyalty Rewards order, will be reimbursed to your account credit.
4. Gifts for 120PV and 190PV tiers will be issued as separate orders upon successful adjustment. The shipping method will follow the original order’s selected delivery method.
  - For orders placed under EC Collection, members are encouraged to request adjustment prior to order collection to ensure that PV promotional items can be processed and collected together.
5. All submissions are final with no changes or cancellations allowed.
6. Please allow 3 working days for processing.
7. Members are advised to contact Member Services (MS) only if there are no updates to their consecutive months or Loyalty Rewards Points from the adjusted order after three (3) working days from the date of submission.
8. All communications regarding this adjustment request will be sent to the email address provided in this form. Members are responsible for ensuring the accuracy of the email address submitted and that they have access to it, and are advised to check their email regularly for updates.
9. This form will close on 30 Jul 2026 at 11:59 PM. All submissions received by this deadline will be reviewed and processed accordingly for the month of Jun 2026.
