2026/27 JPL Trial Form

Cuxton Ascend JPL Trials will take place on the following dates:

@ Leigh Academy Strood, Carnation Road, Strood, Rochester, Kent, ME2 2SX

SESSION 1 : May 23rd, 2026 

SESSION 2 : June 6th, 2026                 

CHECK IN:

- U9's: 9:45am

- U10's: 10:45am

- U11's: 11:45am

FINISH:

 

 - U9's: 11:30am

- U10's: 12:30pm

- U11's: 1:30pm
         

*All players are required to arrive properly dressed for Football training.(Shorts, Socks, Football Boots, Shin-pads)

 

 

Upon the completion of the open trial dates, we will be hosting an invite only trial on Saturday 13th June.

We request that players attend both open trial dates if possible.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Ascend Sports Coaching during the selected activity. In exchange for the acceptance of said child’s candidacy by Ascend Sports Coaching, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Ascend Sports Coaching and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Ascend Sports Coaching, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including Futsal. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

BY AGREEING ABOVE AND SUBMITTING THIS FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT IS EQUIVALENT TO AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
