The undersigned parent/guardian of the above-named participant understands that participation in camp involves risk of injury. I acknowledge and accept all risk associated with camp, which will involve physical exertion and physical contact. I assume any and all risks associated with my child's participation of the camp, including but not limited to, extreme physical exertion, falls, bodily injury due to physical contact, and all such risks being known and appreciated by me. I the guardian attest that my child is healthy to participate in camp activities.
I understand that participation in activities is not covered by college provided insurance. Knowing the risk, and in consideration of being permitted to participate in the camp, do hereby hold harmless and release, wave, discharge and covenant not to sue the Board of Governors or State Fair Community College, its board members, officers and employees from any and all claims of liability on account of death or injury to person or property of the undersigned of any kind of nature whatsoever arising out of, or in any way connected with, the under signs participation in camp, even though the claim or liability may arise out of the negligence or carelessness on the part of SFCC or any third person, whether foreseen or unforeseen, known or unknown. It is understood that in the event of a medical emergency, every effort will be made to contact the emergency contact provided. If I cannot be reached, I hereby authorize State Fair Community College to seek and consent to medical treatment for the participant. I agree to be responsible for any medical expenses incurred as a result of participation in the camp.
The parent and guardian agree that this release and waiver is intended to be as broad and inclusive as permitted by the laws of the state of Missouri and that if any portion hereof is held invalid, it is agreed that the balance, notwithstanding, continue in full legal force and effort.
The parent/guardian further stated that he/she has carefully read the forgoing release and waiver of liability, knows the contests thereof, and has agreed to sign this release and waiver of liability as he owns free act and deed.
