• TEEN COOKING CLUB SAFETY WAIVER AND PROMOTION RELEASE FORM

    Teen Cooking Club is an event at Schlow Centre Region Library and co-sponsored by Centre LGBT+. Once signed, this document releases the library, partners, and staff from liability and grants permission to use photos and video in promotional materials, both online and in print.

  • PROMOTION

  • I, the undersigned, hereby grant to the Schlow Centre Region Library, its officers, representatives, employees, volunteers, and all other individuals or organizations acting in concert with Teen Cooking Club team the right to take photographs of me and my property in connection with the above-identified subject without further consideration or compensation, and I acknowledge the library’s right to crop or treat the photograph at its discretion. I authorize Schlow Centre Region Library, its assignees and transferees to copyright, use and publish the same in print and/or electronically.


    I agree that Schlow Centre Region Library and its partners may use such photographs of me with or without my name (last names of children under the age of 18 will not be used on Internet projects) and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content. 

  • SAFETY

  • I understand and agree that I am participating in the Teen Cooking Club voluntarily and at my own risk. I will not hold Schlow Centre Region Library, its directors, officers, representatives, employees, volunteers, and all other individuals or organizations acting in concert with the Teen Cooking Club team liable for any negligence or alleged negligence or other fault (not including intentional acts) that results in personal injury, death, or property damage during or in connection with the above program. The undersigned, for myself and for my heirs, executors, administrators and assigns, hereby releases and forever discharges Schlow Centre Region Library, its directors, officers, representatives, employees, volunteers, and all other individuals or organizations acting in concert with the Teen Cooking Club team from all such claims. This waiver will be construed according to the law of the Commonwealth of Pennsylvania.

  • THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THE RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, and further agrees that no oral representations, statements, or inducement apart from the foregoing written agreement have been made.
