The Atwater-Merced Alumnae Chapter of Delta Sigma Theta Sorority, Inc. invites you to nominate individuals for the Jazz Under The Stars Community Awards, which recognize outstanding service and leadership within Merced and Stanislaus Counties. Nominees must reside in or provide their primary service within these counties and within the service area of the Atwater-Merced Alumnae Chapter.

We are seeking individuals who have used their powerful voices to make significant, unique, and lasting contributions in their community, workplace, or school—those who have improved lives within their community, demonstrated purposeful action, and served as role models of achievement while providing dedicated service in one or more of the following areas:
• Educational Development
• Economic Development
• International Awareness & Involvement
• Physical & Mental Health
• Political Awareness & Involvement
• Multidimensional Impact (service in two or more categories)
Please complete the nomination form by describing the nominee’s qualifications and accomplishments in the selected category. All contributions should have occurred within the last five years. You may also include supporting documentation such as resumes, letters of commendation, newspaper articles, social media links, or a list of relevant activities.

Award Presentation: September 19, 2026, at the Jazz Under The Stars Event. Selected honorees will receive two free tickets to the event, and two additional tickets at early bird pricing.

Awarded to individuals whose civic leadership echoes like a powerful solo, inspiring democratic engagement and justice.
• Wellness in Tune Award – Physical and Mental Health
For those harmonizing minds and bodies in the community, promoting holistic health and wellness for all.
• The All That Jazz Legacy Award – Multidimensional Impact
Presented to an individual who goes above and beyond across multiple areas, setting the tempo for excellence, compassion, and transformation.
