I agree that, if my application is approved, I am required to submit a certificate of liability insurance for the event. Failure to provide this documentation in a timely manner may affect my application’s position in the overall review process.

I understand that I need to provide a ground cover if using grease.
I understand that dumping or pouring grease on the ground or into drains is strictly prohibited.
I understand that I am responsible for my own trash within my booth area.
I understand that my booth can be shut down for the following reason: a. Featuring items not listed on my application
