This is a new Approval and Workflow process for the 2026-2027 Season

This form is used to facilitate the submission & approval of the below requests. This form is typically filled out by an Association's Hockey Director or Coach with Hockey Director Approval-

8U to 10U play up requests and approvals
12U to 14U recreational play up requests and approvals
Flex Roster
Out-of-State (Youth & Girls) player approval for participation in Colorado (USAH/CAHA sanctioned Hockey)
Medical play down requests and approvals

Medical Play-down Request are approved at the USAH level and initiated at the CAHA Level. As part of the submission, USAH requires substantial documentation to be added to this form, in the uploads section, and as follows:

* Letter From Player's Parents requesting the Medical Play-down & Reason

* Letter for Association President Requesting the Medical Play-down & Reason

* Letter from Player's Doctor Requesting the Medical Play-down & Reason

Medical Play Downs Require the following 3 Documents for review and approval-

1) Signed Letter from Medical Doctor recommending the player compete at a lower level.

2) Signed Letter in support from the Association's Hockey Director in support of the listed player competing at a lower level

3) Signed Letter from the player's parent's acknowledging the request for the player to play-down.
