  • All successful Ambassadors will receive a welcome pack, handbook, badge, and invitation to an in-person Mental Health & Suicide Prevention group training session.

    Please note, you will only be recognised as a Mind Ambassador upon completion of your in-person training session. The training session will be held in Hereford City and will be a group session of up to 20 other applicants, led by Herefordshire Mind's Director Of Services. If successful, you will be offered a choice of 2 dates and times for your training session. Subject to availability. The training dates are Friday 26th June 9am-12:30pm or Monday 29th June 6pm-8:30pm.

    By submitting this form, I confirm that:

    ·        I understand that my information will be stored securely and only used for the purposes of the Mind Ambassador Scheme.

    ·       I understand that I can request to view or delete my data at any time by contacting Fundraising@herefordshire-mind.org.uk.

    ·        I have obtained permission from my employer to take part in this scheme on behalf of my organisation, where appropriate.

    ·        I understand that I can withdraw from this scheme at any time, without giving a reason.

    ·        I understand that I will receive a 3 hour free training session in mental health awareness and be invited to an annual networking event with other Ambassadors.

    ·        I understand that I do not have permission to act on behalf of Herefordshire Mind but can signpost to their services and will demonstrate Herefordshire Mind’s values and key missions.

    ·        I understand that I will not be expected to handle safeguarding concerns. If a safeguarding issue arises, I will follow my own organisation’s safeguarding policy and procedures.

    ·        I understand that only 1 employee per business can become a Mind Ambassador. Herefordshire Mind may make exceptions for larger businesses or organisations.

    • I understand that I can opt-out of receiving communications from Herefordshire Mind at any time, by requesting this in writing. 
    • I am able to attend one of the above training dates. 
  • I agree to take part in or host at least one fundraising event in the next 6 months with the support of Herefordshire Mind to help further support the Ambassadors project.*
