The IDOE requires current proof of Indiana residency for the Choice Scholarship. Please upload one of the following acceptable documents: utility bill, bank statement, credit card statement (dated May 1 or after - only 1 page needed), current lease agreement/mortgage statement, current USPS change of address confirmation, current homeowner's/auto insurance policy/statement, 2025 Form 1040 (page 1 only), or 2025 W2.
