SECTION A: THE CANADA PACIFIC DISTRICT CHURCH OF THE NAZARENE CAMPING PROGRAM AGREES TO:

• Provide criminal record checks for all staff members working with children and teenagers. • Provide appropriately trained first aid personnel for both onsite and offsite activities. • Provide lifeguards as applicable to the organized and structured program of the camp. • Provide appropriate supervision and security for campers throughout their time at the camping venue. • Ensure that proper and appropriate safety regulations and procedures are enforced where vehicular transportation is required. Ensure that accurate recordkeeping is in place to reconcile the number of occupants per vehicle before, during and after any excursion. • Administer medication that may be necessary, provided that a parent/legal guardian has fulfilled their responsibility regarding dosage and other administration details. Further, we agree to ensure that, to the best of our ability appropriate measures will be taken to protect children/teens with known allergies and as well, to provide, to the best of our knowledge, a peanut free environment. • Establish behavioural limits to ensure the safety of all children/teens and to ensure that these limits are clearly defined and consistently maintained for all children/teens. We also agree to ensure that desired behaviour is modeled by the staff through consistent and predictable conduct. We will not knowingly allow a child/teen to physically hurt another person. • Under no circumstances are children/teens subject to corporal punishment, restraint, deprivation or degrading treatment.

SECTION B: AS A PARENT/LEGAL GUARDIAN I AGREE TO:

- 2.1. Waive all claims against the Church of the Nazarene in excess of public liability insurance carried by the organization.
- 2.2. Authorize camp leadership to arrange emergency transportation to a local clinic or hospital in the unlikely event of injury to my child/teen.
- 2.3. Authorize medical personnel to administer care in the event that camp leaders are unable to reach me in a timely manner.
- 2.4. Provide appropriate documentation and dosage requirements in the event that medication must be administered during my child's attendance at camp.
- 2.5. Complete appropriate medical documentation prior to the start of camp that will alert staff to any life threatening allergies.
- 2.6. Allow the taking of all image producing video and photos for purposes of the camp program and for potential future distribution and viewing in local churches, on our website, on our Facebook Group page or for other publicity and publication purposes.
NOTE REGARDING EXCLUSION FROM PHOTO/IMAGE TAKING: In the event that you are in agreement with all of the above requirements except for Item #2.6 and wish for your child/teen to attend camp if an exclusion is in place for this item, please indicate your request during the registration process and appropriate arrangements will be made to accommodate your individual circumstance.

The Cost for KidzCamp are:

Early Bird:                  $450.00 until June 30

Regular Rate:           $460.00 July 1st to July 31

Late Registration:    $475.00 August 1st-August 9

Option #2

1. Print the Registration Confirmation from your email inbox.

2. Write a cheque for the total amount due. Make cheque payable to 'Canada Pacific District'; CAMPS on the memo line

3. Mail Registration Confirmation and cheque to:
