I, the undersigned participant (or parent/guardian if under 18), acknowledge that participation in the Elite Royale Athletics Community Day Kickball Game involves physical activity and potential risk of injury.
I understand and voluntarily accept all risks associated with participation, including but not limited to falls, contact with other participants, and outdoor conditions.
I hereby release, waive, and discharge Elite Royale Athletics, Elite Royale Youth Foundation, its staff, volunteers, sponsors, and affiliates from any and all liability, claims, demands, or causes of action for injury, illness, or damages that may arise from participation in this event.
I also grant permission for Elite Royale Athletics to use photos or videos taken during the event for promotional purposes.
