KCC Health Records Checklist for Nursing (RN/PN) Students
The following blood draw(s), immunizations, and tests are required for all Health Careers RN/PN students. Please attach the appropriate documentation of blood draw, immunization, and test records.
This requirement must be completed before your class starts.Complete the background check form entirely. Pay for the $34 fee in the accounting office (located on 2nd floor). Turn in your completed form and payment receipt in the division office, Room W102.
This requirement must be completed within the first 2 weeks of class. You cannot participate in demos or clinical without a physical.Physicals must be documented on a KCC physical form. The physical can be performed anywhere.Placement in a clinical/lab within a health career program is conditional in that you must be physically capable and emotionally stable to perform the essential functions required in the specific program with or without reasonable accommodation and be free from communicable diseases in the opinion of your physician. A medical examination by a legally qualified healthcare provider is required. The examination must be complete and on file in the Health Careers Division within the first 2 weeks of class. Physicals cannot be older than four months from your first day of classes.
This requirement must be completed before class starts.Your drug screen must be completed at one of two facilities. No paperwork is needed to take with you to the facility to complete this requirement. The facility will send the results to KCC. This requirement cannot be completed more than four months prior to the start of your program of course.

*If drug screen is completed at this site, you must sign a consent to release results to KCC Health Careers before leaving the clinic. *
Two-Step TB (within the last year) Must be a TWO step TB test (Four trips to the facility of your choice are required to complete this lab test and can take up to 2 weeks to complete) * Results must be recorded in "mm" induration. If TB test is from your employer, documentation needs to be on company letterhead, or the form used at the facility. *If you test positive, contact the county health department for the screening process. You will also need a chest Xray. *QuantiFERON Gold TB test acceptable instead of 2-step TB test. This is a blood draw and can be costly if insurance doesn't cover it.
Tetanus Booster (must be within the last 10 years) Also called Tdap or TdVaricella (2-shot series, these can be from when you were a child) *Varicella titer only required if student does not have documentation of Varicella vaccines. If your titer comes back negative/non-immune, you will need to start the 2-shot series. You must turn in documentation of the 1st shot to be able to attend clinical.MMR (2-shot series, these can be from when you were a child) *MMR titers only required if student does not have documentation of MMR vaccines. If your titers come back negative/non-immune, you will need to start the 2-shot series. You must turn in documentation of the 1st shot to be able to attend the first day of clinical.Hepatitis B (3-shot series, these can be from when you were a baby) *Titer only required if student does not have documentation of Hepatitis B vaccines or if recommended by the student's healthcare provider. If your titer comes back negative/non-immune, you will need to start the 3 shot series. You must turn in documentation of the 1st shot to be able to attend clinical.Influenza (Fall & Spring semester students)COVID-19 *At this time, COVID-19 vaccination is not a requirement of the nursing courses but needs to be tracked for reporting to the clinical facilities. Please turn in a copy of your COVID-19 card or record of any booster shots you may have received.
The only CPR cards that are accepted are from American Heart Association or The Red Cross. You cannot do an online only class. There must be a hands-on, in-person demonstration portion to your CPR class.
