REPAIR AFFAIR - VOLUNTEER SIGN-UP SHEET

Supplies and materials will be at each job site on the day of the Repair Affair event. Volunteers will need to bring some tools and equipment of their own to help with their project. Examples of items needed: cell phones, ladders, lawn mowers, weed eaters, rakes, drills, hammers, saws, extra paint brushes, etc.

I, on behalf of myself and/or my minor child, hereby releases and holds harmless sponsoring organizers of the Repair Affair event, their officers, employees, volunteers, and contributors from all liability for damages or injuries suffered by me and/or my minor child in conjunction with participation in the Repair Affair event. The undersigned also acknowledges that organizers of the Repair Affair event do not maintain liability insurance coverage pertaining to the Repair Affair event. In the event that a Repair Affair client alleges damage(s) to his/her person/property related to the undersigned volunteer's actions, the undersigned is not covered under their insurance policy. The organizers of Repair Affair event may use my photograph for promotional purposes (e.g. print advertisement, social media, etc.) pertaining to the Repair Affair event or future Repair Affair events.
