TR1BE Volleyball — Tryout Liability Release & Waiver of Claims
Tryout Location: 1632 Little Star Drive, Cheyenne, WY 82009 Tryout Dates: June 15–18, 2026 · 6:00 PM – 8:00 PM
I, the undersigned parent or legal guardian ("Guardian") of the above-named minor player ("Participant"), do hereby acknowledge, agree, and represent the following:
1. Assumption of Risk. I understand that participation in volleyball tryouts involves inherent risks including but not limited to sprains, strains, fractures, concussions, and other bodily injury. These tryouts will be held at a private residential home gym at 1632 Little Star Drive, Cheyenne, WY 82009. I voluntarily accept and assume all such risks on behalf of the Participant.
2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue TR1BE Volleyball, its coaches, organizers, volunteers, the property owner(s), and all associated individuals ("Released Parties") from any and all claims, damages, costs, or liability arising out of or related to the Participant's involvement in tryouts, including claims arising from the negligence of the Released Parties.
3. Medical Authorization. I authorize the Released Parties to obtain emergency medical treatment for the Participant should the need arise, and I accept financial responsibility for any such treatment.
4. Deposit Policy. I understand that if the Participant is selected for the team, a non-refundable deposit will be required to secure their spot. If the Participant commits to the team and later withdraws, the deposit will not be refunded.
5. Photo & Media Release. I grant TR1BE Volleyball permission to photograph or record the Participant during tryouts for team and promotional purposes.
6. Health Confirmation. I confirm the Participant is in good physical health and has no known medical conditions that would prevent safe participation. Any relevant conditions have been disclosed in this form.
By signing below, I confirm that I am the parent or legal guardian of the Participant, that I am 18 years of age or older, and that I have read and fully understand this agreement. A typed name and/or drawn signature constitutes a valid electronic signature under applicable e-signature laws.
