This survey is intended to be taken by people who live, work, or learn in Brown County. It was created by Beyond Health, which is a collaborative group including local health departments, health systems, and other health-focused organizations.
Data from this survey will be used in the Community Health Assessment and will help us decide which health priorities to focus on.
You must be 18 years or older to take this survey.
This survey will take about 5-10 minutes to complete. All answers are completely anonymous. After submitting the survey, you will have the option to enter a drawing to win a $50 gift card (25 total). The gift card drawing is not connected to survey responses in any way.
