    Celebrate the End of the Year with an Unforgettable Field Trip!

    Ready to wrap up the school year with something actually fun? Join us for a high-energy field trip that combines art, games, food, and time with your friends. The perfect way to celebrate everything you’ve accomplished this year!

    We’ll kick things off with a visit to the Jordan Schnitzer Museum of Art, where you’ll explore powerful, creative works from around the world. This isn’t a boring, silent museum trip—you’ll get to share your thoughts, ask questions, and connect what you see to your own life and experiences. Art workshop at the museum is included, and a chill lunch on campus grounds.

    Then, we’ll switch things up and head to Round1 Bowling & Arcade for a full-on celebration. Get ready for:

    • Unlimited arcade gameplay!
    • Bowling with your crew!
    • Sing your fav jams in karaoke!
    • Ping pong AND billiards!
    • Pizza! Wings! Takoyaki! Gyoza!
    • And a chance to win credits for the Victory Zone!
      • where you can use your claw machine skills to win your favorite anime merch and more!

    Your head will be spinning with all of the options! Hope to see you and your friends there! Celebrate the end of the year in a way that actually feels rewarding. This trip is all about you, your efforts, and your growth, so let's make sure you end the year on a high note!

    Consent to Emergency Care and Relevant Information:

    In the event of an emergency, if reasonable attempts to contact persons listed are unsuccessful, I give consent to the NAYA Family Center to transport myself or my child to the doctor or to the Emergency Medical Department of the nearest hospital. If my doctor cannot be contacted, all medical treatments deemed necessary by the attending licensed physician, including examinations, anesthesia, operations, and diagnostic procedures, may be administered. I further acknowledge that I will be responsible for all fees and costs associated with the medical treatment that are not covered by my insurance.

    Consent for Transportation in NAYA Vehicles and NAYA Staff Vehicles:

    I understand that NAYA Family Center programs and activities may include local and long distance travel, and give permission to the NAYA Family Center to provide transportation. The participant understands and agrees to abide by the following rules with being transported.

    • All participants must be responsible for wearing seatbelts at all times.
    • All children under 80 lbs. are required to be properly placed in a child seat or booster seat.
    • Appropriate conduct will be mandatory when being transported.
    • Smoking is not allowed in NAYA Family Center vehicles or personal staff vehicles when being transported.

    A participant may be subject to suspension of transportation privileges if found in violation of any rules of transportation, or for disobeying the NAYA Family Center staff providing the transportation.

    Field Trip Agreement:

    As the Parent/Guardian of {studentName}, I give permission for them to leave NAYA for this trip. By signing this agreement, I acknowledge that NAYA Family Center has my permission to transport my child and include my child in all of the field trip activities unless otherwise specified. In the event of an emergency, the signature attached below hereby consents to any and all medical and surgical treatment, including anesthesia and operations, which may be deemed advisable by their physicians and surgeons to perform all examinations. I also agree that the patient, when admitted, is to remain in the hospital until their physician recommends the patient’s discharge. NAYA Family Center will not be held liable for any injuries or accidents caused to your child if they choose not to follow NAYA Family Center staff directions or rules.

    Round1 Expectations and Agreement

    By attending this field trip to Round1, I agree to follow all safety rules and respect all equipment and spaces. I understand that:

    • I will follow all bowling safety rules, including:
      • Not crossing the foul line (slippery surface).
      • Not throwing or lofting balls dangerously.
      • Only one person on the lane approach at a time (unless assisting a child).
      • Wear proper bowling shoes at all times.
      • Keeping food and drinks off the lane area.
    • I will use arcade games and equipment properly, without hitting, shaking, or misusing machines.
    • I will follow all rental equipment rules, including: Returning equipment on time and in good condition.
    • Understanding that damaged or lost equipment may result in charges.
    • I will not run, push, or engage in unsafe behavior in activity areas.
    • I will follow Round1 and NAYA staff instructions and all posted facility rules at all times

     

     I understand that I am responsible for treating all equipment and spaces with care. Unsafe behavior, misuse of equipment, or failure to follow rules may result in removal from activities and/or charges for damaged or lost equipment.

    Expectations and Agreements at UofO's Jordan Schnitzer Museum of Art

    By attending this field trip to the Jordan Schnitzer Museum of Art, I agree to follow all museum rules and expectations. I understand that:

    • I will stay with my group, remain attentive, and follow all instructions from staff and group leaders.
    • I will help maintain a respectful, focused environment by avoiding disruptive behavior and supporting group participation.
    • I will keep a safe distance (at least 12 inches) from all artwork and will not touch any art or displays.
    • I will not run, engage in horseplay, or engage in any unsafe behavior in the galleries.

    Respect for Artwork & Space

    • I will not bring food, drinks, or liquids into gallery spaces.
    • I will store large bags, backpacks, and wet items (coats/umbrellas) in designated areas
    • I will only use pencils (no ink pens) when writing or sketching.
    • I will not use tripods or selfie sticks inside the museum.
    • I will only take photos where allowed, without flash or video recording, and will check artwork labels for restrictions.

     

    I understand that I am responsible for respectful behavior and protecting the artwork. Failure to follow these expectations may result in removal from the museum and/or art workshop.
