  •  PATCH stands for Providers and Teens Communicating for Health. At PATCH, we believe that health care practices, programs, and policies should be developed with young people rather than just for them.

    In this program, we rely on YOU to be the expert and share what you know about being a teen, like:

    • What are you and your peer’s concerns about heath & health care?
    • What are things that make you comfortable or uncomfortable accessing health care services?
    • How much do you or your peers know about your health care rights as a teen?
    • Where do you and your peers find health information?
    • What information and resources do you and your peers need?
    • And so much more!

    Please review all the expectations below and check the box for each of them before submitting your application.
  • I understand that as a PATCH Teen Educator or Teen Consultant I must:

    • Commit to this position for the whole program year.
    • Attend all of the mandatory in-person training during dates shared in the job description.
    • For Teen Consultants Only: Participate in virtual evening meetings two times each month.
    • For Teen Educators Only: Participate in evening in-person meetings two times each month.
    • For Teen Educators Only: Find transportation to and from all workshops and meetings (some travel assistance may be available).
    • Be in regular contact with program staff via email and other modes of communication.
    • Represent myself and the organization responsibly.
    • Be respectful of my fellow team members, supervisor, and other individuals involved in the program.
    • Be open minded and excited about working with people who may be different from myself.
