This medical release form is to be filled out completely and submitted with your application. In the event of an emergency, medical expenses are your responsibility. By completing the following form, you are releasing the Lutherhaven Board, Camp Director(s), Camp Staff, and Olympic Lutherhaven from any liability.

I will be volunteering at camp part-time and recognize that I will be paying for my camper's registration fees.I will be volunteering full-time (including sleeping on site during the week of camp in my own camper/tent or in the main lodge) and will have my children at camp (camp fees waived for up to 2 children--please complete the camper registration form ASAP to reserve their spots).I will be volunteering at Olympic Lutherhaven, but do not have any children attending camp.
