I understand that, as part of the volunteer process with Girl Let Go Inc, I am required to complete a background screening prior to approval. I understand that I will receive a secure link to complete my background screening through a third-party provider. I am responsible for accurately entering my information, completing all required forms, and submitting payment at the time of completion.

I acknowledge that I am required to provide a valid government-issued photo ID, such as a driver’s license or state ID, and that the ID must not be expired. I also acknowledge that I am required to disclose any and all names I have legally been known by, including aliases, maiden names, and prior legal names.

I understand that failure to disclose this information, or providing false or incomplete information, may result in denial or termination of volunteer participation. I understand that completion of a background check does not guarantee acceptance as a volunteer.
