WAIVER OF LIABILITY & PARTICIPANT AGREEMENT
Little Champs Sports Academy ("Program") is a youth sports and movement program for children ages 2–5. This program requires a parent or guardian to actively participate alongside their child during all sessions. By enrolling, you agree to the following:
Parent Participation Acknowledgment
I understand that Little Champs Sports Academy is a parent-and-child participation program. I confirm that I will actively participate alongside my child during all sessions. I confirm that I am physically able to participate in light physical activity including running, jumping, and movement-based games, and I agree to engage safely and responsibly throughout each session.
Assumption of Risk
I understand that participation in physical activity carries inherent risks including but not limited to falls, collisions, and minor injuries. I voluntarily accept these risks on behalf of both my child and myself as a participating parent/guardian. I acknowledge that these risks exist even when the program is conducted with reasonable care.
Release of Liability
I agree to release and hold harmless Janet Holyoak and Little Champs Sports Academy from any claims, damages, or injuries arising from my child's participation AND my own participation as an accompanying adult in the program, except in cases of gross negligence. This release applies to any injuries, losses, or damages incurred during program sessions or while on the premises.
Medical Authorization
In the event of an emergency involving my child, and if I cannot be reached, I authorize program staff to seek emergency medical care for my child. I understand that as a participating adult I am responsible for seeking my own medical care if needed.
Program Policies
- Classes are $125/month for four sessions, due before the first class of each month.
- No refunds for missed classes. Make-up sessions will be offered when possible.
- A parent or guardian must remain on-site and actively participate for the duration of every session — drop-offs are not permitted.
- Participants should wear comfortable athletic clothing and closed-toe shoes.
- Little Champs reserves the right to cancel a session due to weather or unforeseen circumstances. Make-up sessions will be offered when possible.
- Little Champs reserves the right to dismiss a participant from the program for behavior that compromises the safety or enjoyment of other participants.
