By signing this form, I acknowledge and agree that: 1. Assumption of Risk I understand that participation in sporting activities involves inherent risks, including the risk of injury or illness. I voluntarily accept these risks. 2. Release of Liability I release and hold harmless Next Level Generation Sport, its staff, volunteers, sponsors, and affiliates from any liability for injury, illness, accident, or loss arising from participation, except where required by law. 3. Personal Property I understand that Next Level Generation Sport is not responsible for any loss, theft, or damage to personal belongings. 4. Supervision Requirement I acknowledge that a parent or legal guardian (18 years or older) must remain present at all times to supervise participants under 18. 5. Medical Responsibility & First Aid Consent I confirm that I am responsible for managing my/my child’s medical conditions and will ensure necessary medication is available at all times. I consent to staff administering first aid and, if necessary, seeking emergency medical treatment, including ambulance services. 6. Behaviour & Code of Conduct I agree that I/my child will follow all instructions from staff and behave appropriately. I understand that failure to comply may result in removal from activities. 7. Photography & Media Consent I acknowledge that photos and videos may be taken during activities. - I consent to the use of images/videos for promotional purposes - While reasonable efforts will be made to accommodate requests to avoid capturing specific individuals, this cannot be guaranteed. 8. Event Changes & Cancellations I understand that events may be modified, postponed, or cancelled due to weather, safety, or unforeseen circumstances. 9. Health & Safety I agree not to attend if unwell and to follow any applicable health and safety directions. 10. Insurance Acknowledgement I understand that personal accident insurance is my responsibility unless otherwise stated. 11. Privacy I consent to the collection and use of personal information for registration, safety, and communication purposes in accordance with applicable privacy laws.
