BELLINGHAM SKILLZ ACADEMY SUMMER CAMP WAIVER & RELEASE OF LIABILITY
By registering my child for Bellingham Skillz Academy Summer Camp, I acknowledge and agree to the following:
1. Assumption of Risk
I understand that participation in soccer training and related camp activities involves inherent risks, including but not limited to falls, collisions, physical exertion, heat-related illness, and other injuries that may occur during training, games, drills, or camp activities. I voluntarily assume all risks associated with my child’s participation.
2. Medical Authorization
I certify that my child is physically fit to participate in camp activities. In the event of illness or injury, I authorize Bellingham Skillz Academy staff to seek emergency medical treatment for my child if I cannot be reached immediately. I understand I am responsible for any medical expenses incurred.
3. Release of Liability
I release and hold harmless Bellingham Skillz Academy, its owners, coaches, staff, volunteers, affiliates, and facility partners from any and all claims, liabilities, damages, or causes of action arising out of or related to my child’s participation in camp, including injuries, accidents, or loss of personal property, except in cases of gross negligence or willful misconduct.
4. Conduct Expectations
I understand participants are expected to behave respectfully toward coaches and other players. Bellingham Skillz Academy reserves the right to remove a participant from camp without refund for unsafe behavior, bullying, repeated misconduct, or disruption.
5. Hydration & Heat Safety
I understand summer camp takes place in outdoor conditions and I am responsible for ensuring my child arrives properly hydrated, with water, appropriate footwear, shin guards (if required), sunscreen, and any necessary medications.
6. Photo & Video Release
I grant permission for Bellingham Skillz Academy to use photographs or videos of my child taken during camp for instructional, promotional, website, or social media purposes without compensation.
7. Refund Policy
I understand camp registration fees are non-refundable except in cases of camp cancellation by Bellingham Skillz Academy or as otherwise communicated.
8. Agreement
By checking the box below and submitting this registration, I confirm that I have read, understand, and agree to this Waiver and Release of Liability on behalf of myself and my child.
