Date(s) and location of activity: July 26th - July 30th at 6 pm at Hillside Baptist Church.
Description of activity: There will be preaching and teaching based off of the Bible. There will be phsyically active indoor and outdoor games, water games, running, throwing, songs, crafts, etc. There will also be snacks including but not limited to ice cream sandwiches, Rise Krispie treats, goldfish crackers, Oreos, and potato chips.

I acknowledge that participation in the activity described above involves risk to the participant (and to the participant’s parents or guardians, if the participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage, and financial damage.
In consideration for the opportunity to participate in the activity described above (the “activity”), the participant (or parent/guardian if the participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the activity. The participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during the activity or during transportation to and from the activity, as well as for any medical treatment rendered to the participant that is authorized by Hillside Baptist Church or its agents, employees, volunteers, or any other representatives (collectively referred to as the “activity sponsor”). Further, the participant (or parent/guardian) releases and promises to indemnify, defend, and hold harmless the activity sponsor for any injury which my child has now or which may arise in the future arising directly or indirectly out of the described activity or transportation to and from the activity, whether such injury arises out of the negligence of the activity sponsor, the participant, or otherwise.
I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Missouri, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital.
I further state that I have carefully read the foregoing release and know the contents thereof, and I sign the release as my own free act. This is a legally binding agreement which I have read and understand.
If the undersigned minor’s behavior is such that it endangers the welfare of others, Hillside Baptist Church or its adult sponsors have our permission to send said child home or to exercise such non-corporal discipline as they deem appropriate. If said child is sent home, I further agree to pay the transportation costs for the trip home.
If a dispute over this agreement or any claim for damages arises, the participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the participant (or parent/guardian) and the activity sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution in accordance with the rules of the American Arbitration Association.
Photo Release: I hereby confer that Hillside Baptist Church has my permission to use any video/photo they have taken of me, or family members directly under my care in any publication (website, printed and electronic material, etc.) for the purpose of promotion or advertising. I hereby relinquish any right that I may have to examine or approve any part whatsoever of the completed product or products that may be used in conjunction therewith or the use to which it may be applied.
