Induction modules - Supervisor form

Use this form if you are a supervisor setting up induction access for your department.

Once submitted:

• We will create your supervisor account (if you don’t already have one)
• You will receive a unique registration link to share with your inductees
• Inductees can then register themselves and will be linked to your hospital group
  
  

Important information

• You only need to register once per hospital/department
• Supervisor accounts remain active unless you request deletion
• Please complete all fields to avoid delays

• Inductees must register themselves using this link
• I am responsible for distributing access within my department

 

What happens next?

• We will process your request and create your supervisor account
• You will receive your inductee registration link by email
• You can then share this with your team
