CM Play Photography Swim Athlete Release 🏊‍♂️✨

This Photo & Media Release (“Release”) is entered into between CM Play Photography LLC (“Photographer”) and the undersigned Parent/Guardian (“Parent”) on behalf of the minor athlete named below (“Athlete”). This Release applies to photography and videography conducted at any Ohio Swimming Inc. (OSI)–sanctioned event and any Ohio High School Athletic Association (OHSAA)–sanctioned swimming event where the Photographer is approved to photograph.

 
1. Consent to Photograph
I give permission for CM Play Photography LLC (“Photographer”) to photograph my Athlete at any OSI‑sanctioned or OHSAA‑sanctioned swimming event where the Photographer has been approved for deck access by the host club, host school, or meet director. All photography will be conducted in accordance with SafeSport, facility rules, and host‑organization policies.

Acknowledgment of Meet Photography Notice
I understand that OSI‑ and OHSAA‑sanctioned events may include general photography or videography notices in their meet information packets. This Release governs the specific rights granted to CM Play Photography LLC regarding the capture, use, sale, and licensing of images.

 
2. Rights Granted to Photographer (Commercial & Editorial)
Parent grants Photographer a perpetual, worldwide, irrevocable, royalty‑free license to:

• create, edit, reproduce, and display images of the Athlete
• sell images to families, athletes, and individuals
• license images to third parties (including sponsors, vendors, publications, and organizations)
• use images for Photographer’s own commercial, promotional, portfolio, and marketing purposes

Parent acknowledges that Photographer’s commercial use may include advertising, promotional materials, digital and print media, and licensing to third parties. No compensation is owed to the Athlete or Parent for any such uses.

 
3. No Obligation to Use
Photographer is not required to use any image of the Athlete. Parent understands that images may appear incidentally in group or background scenes.

 
4. Assumption of Risk
Parent understands that photography occurs in an active sports environment and agrees that Photographer is not responsible for injuries, accidents, or incidents unrelated to the act of photography.

 
5. Release of Claims
Parent releases and holds harmless Photographer, Photographer’s agents, the host club or school, and event organizers from any claims related to:

• the capture, use, display, sale, or licensing of images
• invasion of privacy
• right of publicity
• unintentional background capture

This Release applies to the Parent/Guardian and the Athlete.

 
6. Validity Period
This Release remains valid for all OSI‑sanctioned and OHSAA‑sanctioned swimming events where CM Play Photography LLC is approved to photograph, until revoked.
