The information below will help you complete the nomination questionnaire and application process. You may nominate an individual or organization.
- An individual is a New Hampshire oral health professional, educator, and/or advocate for public oral health services.
- An organization is a nonprofit entity working in New Hampshire whose mission includes improving the oral health of the people of New Hampshire.
**Members of the NHOHC Board are not eligible for nomination. Self-nominations will not be accepted.
Application Procedure
-Complete and submit this nomination questionnaire plus 2 letters of recommendation. You will be prompted in the questionnaire to attach the letters.
Award Criteria and Examples
This award honors Dr. Skip Homicz and his oral health legacy of commitment, service, mentorship, advocacy, and leadership in serving the people of NH. The criteria below may help you in your submission.
Individual Characteristics
· Demonstrates exceptional commitment to, and impact on, the oral health of the people of New Hampshire.
· Is recognized as a leader in the oral health field.
· Serves as a role model to others.
· Is a passionate advocate for improved access to oral health services.
· Provides, advocates, and/or supports compassionate, non-judgmental care to all patients.
· Demonstrates a commitment to learning and working in partnerships to advance oral health.
· Seen as an ambassador in the public oral health field by recruiting, teaching, and training others to serve.
Organizational Criteria
· Has a track record of providing excellent oral health services, particularly to underserved populations.
· Is exceptionally committed to improving access to oral health services for the people of New Hampshire.
· Is recognized as a leading advocate for improving oral health services.
· Is regarded as a state or community leader in the oral health field.
· Implements and/or advocates for oral health integrated with overall health services.
**DEADLINE: Submission must be made by 11:59 p.m. Friday, August 14, 2026.
The application must also include two current letters of support from others that address the contribution the nominee has made and why the nominee deserves this recognition. You will be prompted to attach the letters within this nomination questionnaire.
