By signing below, I acknowledge and agree to the following:

👀 Supervision Responsibility

I understand that Bright Young Minds is a parent-supervised facility, and I am responsible for supervising my child(ren) at all times while in the activity center.

🛝 Assumption of Risk

I understand that participation in activities at Bright Young Minds STEAM Activity Center involves inherent risks, including but not limited to:
- Slips, trips, and falls
- Collisions with other children or objects
- Use of climbing structures, play equipment, and building materials
- Handling small objects that may pose a choking hazard for younger children
- Participation in sensory, art, and STEAM activities involving various materials
- I acknowledge that these risks may result in injury, including minor injuries (such as bumps, bruises, or scrapes) and, in rare cases, more serious injury.
I understand that Bright Young Minds provides a space designed for active, hands-on play and exploration, and that injuries can occur even when all reasonable safety precautions are followed.

I agree that:
- My child(ren) participate voluntarily in all activities
- I am responsible for determining whether activities are appropriate for my child’s age, ability, and development
- I will actively supervise my child(ren) and ensure they are using equipment safely
- I further acknowledge that:
  - Bright Young Minds is not responsible for injuries resulting from misuse of equipment, lack of supervision, or failure to follow posted rules
  - By signing this agreement, I voluntarily assume all risks, known and unknown, associated with my child’s participation in activities at Bright Young Minds STEAM Activity Center.
🤝 Behavior Expectations

I agree that my child(ren) will follow all posted rules, including:
- Kind and respectful behavior
- No rough or unsafe play
- Proper use of materials and equipment
- I understand that repeated unsafe or disruptive behavior may result in removal from the center.
⚠️ 3-Strike Safety Policy

I acknowledge and agree to the Bright Young Minds 3-strike policy regarding bullying or dangerous behavior. I understand that:
- Continued unsafe behavior may result in dismissal for the day
- Repeated incidents may result in suspension of access
🧼 Health & Illness Policy

I agree that my child(ren) will not attend if they are sick or showing symptoms of illness.

🍎 Food & Snack Policy

I understand that:
- Food and drinks must be consumed in designated areas only
- All snacks and drinks must be paid for before being opened or consumed
🧸 Care of Facility

I agree that my child(ren) will help respect and care for the space, materials, and equipment.

⚖️ Liability Waiver

To the fullest extent permitted by law, I release and hold harmless Bright Young Minds, its owners, staff, and affiliates from any liability, claims, or damages arising from participation in activities at the center.
