I. Assumption of Risk: I understand and acknowledge that the dangerous activity(s) I am voluntarily choosing to participate in, such as paintball, airsoft, nerf wars, and laser tag is dangerous and may involve risk of injury. That injury can be temporary or permanent, death, and/or property damage. I understand and acknowledge that injuries can occur while participating in activities run or hosted by UBG Airsoft, LLC (UBG) including but not limited to strains, fractures, partial or total paralysis, heart attacks, cuts, abrasions, spinal injuries, loss of hearing, seizures, permanent loss of sight, heat stroke, death or other ailments or sicknesses. I further understand that I may be traversing hazardous terrain and may be exposed to inherent dangers and hazards in buildings and out such as breached nails, sharp edges, loose nails and screws, unstable structures, falling tree branches or structures, and wet or frozen puddles and floorboards. I further understand that I may come in contact with wildlife that may be hazardous to my health. I understand that UBG makes no representation as to the condition or safety of the premises and that all use of the premises by the party is being taken "AS IS". I hereby do voluntarily assume all risks and dangers that may occur pursuant to my use of the premises and participation in activities on the premises, including the risk of injury, death, or property damage, even if caused by the negligence of the company, its owners, employees, officers, and agents.

II. Safety Rules: I understand and acknowledge that UBG airsoft has posted rules regarding the participation in all activities on the premises. I understand that it is my responsibility to ensure adherence to all the posted rules and regulations. I further acknowledge that my failure to fully understand and adhere to these rules and regulations is in no way the fault or responsibility of UBG and hereby do release UBG and its staff, owners, officers, employees, and agents from any claims for injuries to my person or property. I acknowledge that it is my responsibility to use the proper safety equipment to protect myself and understand that even when using this safety equipment properly injury or death may occur. My failure to adhere to these rules may result in the company, its owners, employees, officers, and agents asking me to leave the premises. If asked to leave the premises due to a violation of a posted rule, I understand that I shall not receive a refund for any amounts spent to participate.

III. Indemnification: I hereby agree to defend, indemnify, and hold harmless UBG, its owners, employees, officers, and agents from and against any losses, damages, actions, suits, claims, judgments, settlements, awards, interest & penalties, expenses and costs of any kind for any personal injury, loss of life, or property damage in perpetuity.

IV. Government Law: This release will be governed and interpreted in accordance with the laws of the Commonwealth of Massachusetts.

V. Release of Liability: I hereby agree that myself, heirs, representatives, and assigns shall fully and forever discharges and release UBG Airsoft, LLC, its affiliates, successors, employees, owners, agents, and officers from any and all claims I may have now or hereinafter obtain for any injury, illness, disability, death, and damage to personal property caused in part or in whole by the actions, inactions, and negligence of third parties, UBG, its assigns, staff, officers, owners, and agents. I understand that this release is intended to be a complete release, and as much as allowed by law of any responsibility the released parties may have.

VI. Media Release: I understand that UBG airsoft or third parties may be taking pictures while participating or watching activities on the property. I hereby agree to give UBG full rights to any pictures and videos taken while I participate in the above activities to use in their sole discretion without compensation of any kind in perpetuity.

VII. Medical Treatment Release: I hereby authorize UBG to secure, and I consent to any medical treatment that may be given to me should the company or its employees determine in their sole discretion that I need medical care. I fully accept all costs related to my medical treatment including transportation costs. I release UBG from all liability that may happen during any treatment or transport from UBG or responding medical personnel. If any personal medical issues are present, it is my responsibility to take that into consideration prior to participating in activities on the property and make responding medical personnel aware.

VIII. Entire Agreement: I understand and acknowledge that this waiver is the entire agreement between the parties and supersedes any prior agreement of communication between the parties whether written or oral, electronic or otherwise. No change, modification, amendment, or addition to this agreement shall be valid unless in writing signed by both parties and/or their authorized representatives.

IX. Minor Child and Parent and/or Guardian Acknowledgment: Read this form carefully and completely, and sign this waiver acknowledging the following: I, as participating minors parent and/or guardian understand the nature and extent of the above mentioned activities and hereby attest that the minor child is capable and qualifies to participate in these activities, however dangerous. The parent and/or guardian agrees to all the above provisions on behalf of themselves and the minor including but not limited to agree to hold UBG, its assigns, staff, officers, owners, and agents harmless for any actions, inactions, and/or negligence on their part. Minor child's parent and/or guardian hereby assumes all potential risks for participation in the above mentioned activities on behalf of the minor. You are hereby agreeing to let your minor child participate in an inherently dangerous activity freely and voluntarily.

I HEREBY ACKNOWLEDGE AND REPRESENT THAT I HAVE FULLY READ AND UNDERSTOOD EACH OF THE ABOVE PROVISIONS AND THAT I AM GIVING UP SIGNIFICANT LEGAL RIGHTS THAT I MAY HAVE NOW OR IN THE FUTURE, INCLUDING THE ABILITY TO SUE THE COMPANY. I ACKNOWLEDGE THAT I AM AT LEAST 18 YEARS OF AGE AND FULLY COMPETENT TO UNDERSTAND THIS AGREEMENT. I HEREBY EXECUTE THIS AGREEMENT VOLUNTARILY BY SIGNATURE BELOW.
