SUMMER 2026 — LIABILITY WAIVER & PARENTAL CONSENT

Please read this document carefully before signing. This waiver applies to all athletes ages 6-15 in the Kingdom Elite Summer 2026 program. A signed waiver is required before tryouts on May 23, 2026. By signing, you confirm you are the legal parent or guardian of the named child.

I understand and acknowledge that cheerleading, stunting, tumbling, and related physical activities carry inherent risks of injury, including but not limited to sprains, strains, fractures, and concussions. I understand that McKenzie Robinson and Jaiden Goins are experienced coaches operating a recreational program and are not certified professional trainers. I acknowledge that safety mats and reasonable precautions are always provided and prioritized. I voluntarily accept these risks on behalf of my child.

In consideration of my child's participation in Kingdom Elite Cheer & Stunt, I, as parent or legal guardian, hereby release, discharge, and hold harmless McKenzie Robinson, Jaiden Goins, and any associated volunteers from any and all claims, demands, losses, or liability arising out of or related to my child's participation in program activities — including practices, performances, and the Summer Kingdom Elite Trip — except in cases of gross negligence or intentional misconduct.

In the event of an emergency or injury during program activities, I authorize McKenzie Robinson and Jaiden Goins to seek appropriate medical care for my child, including contacting emergency services. I give permission for coaches to administer medication that I have provided, or to assist in obtaining medical attention as needed. I confirm that my child is physically able to participate and has no known health conditions that would prevent safe involvement. Any exceptions must be disclosed below.

I give permission for my child to be picked up, dropped off, or otherwise transported by McKenzie Robinson and/or Jaiden Goins for program activities, including practices, local performances, and the Summer Kingdom Elite Trip. I understand that all reasonable care will be taken during transportation.

I understand that participation in the Kingdom Elite Summer 2026 program carries a seasonal fee, full details of which will be provided upon selection. This fee is due in full only if my child is selected for the official team following tryouts. The fee covers program materials including a custom gold bow, poms, practice location and the Summer Kingdom Elite Trip. I agree to pay the fee in full upon my child's selection.

I understand that parents are responsible for providing: white indoor cheer shoes, a water bottle, and a practice bag.

I understand that Kingdom Elite maintains a strict attendance policy. Athletes are allowed a maximum of 3 missed practices per season. Excessive absences may result in removal from the team. I agree to communicate any anticipated absences to the coaches in advance.
