• 2026/2027 Fannin CLC Personal Information and Parent Form

    Please fill out the form below with your personal details and provide your electronic signature at the end of each section to complete the application. *By signing this permission form, you acknowledge that if your child is enrolled in the Fannin CLC Connections class, this permission will remain valid for the duration of the 2026-2027 school year, including any return enrollment.

  • Permissions

    By signing each section below, I acknowledge and agree to the following permissions on behalf of my child.

  • 1. Permission to Participate in CLC
    I give permission for my child to participate in the released time religious program provided by the Fannin Christian Learning Center (CLC). I understand that attendance in this program occurs during school hours and is a connections class, a Bible course taught off school property. 

  • 2. Media Release
    I consent to the use of my child’s name, voice, and likeness in photos, video, audio, and print that may be used in social media, promotional material, public presentations, or other forms of media representation by the CLC and its affiliates.

    I release the Fannin CLC, its staff, and associated organizations from liability related to the use of such media.

  • 3. Medical & Educational Information
    I have provided any known allergies, medical conditions, or medications below. Your signature at the end of this document gives permission for the CLC to work with Fannin County Public School System to access this information to support my child during the school year.

  • 4. Participation & Grading Policy
    I understand that my child will:

    Take weekly Memory Verse Tests (even if school is canceled — test will be given upon return)
    Complete daily assignments and weekly quizzes

  • 5. Communication & Notifications
    I will ensure that I am available to communicate and agree to receive updates through email. 

    I understand I will receive a progress report via email or a physical paper sent home with the student every nine weeks.

  • 6. Transportation Permission

    I give permission for my child to be transported between the CLC Middle School Campus and Fannin County Middle School by CLC staff using a van or golf cart whenever staff determines that transportation is necessary. This may include, but is not limited to, weather-related situations, student or staff needs, scheduling adjustments, or other circumstances requiring relocation. I understand that this transportation involves my child leaving the CLC campus and traveling to another school facility.

    When feasible, transportation will occur via the “Peace Path” (the gravel drive connecting the two campuses) to avoid travel on the highway. However, please note that alternate routes, including the highway, may be used when conditions—such as significant rainfall—make the gravel route unsafe or impractical.

    By signing this document, I grant permission for my child to be transported using these methods between the CLC and the middle school as needed, including for early arrival, late arrival, or participation in school-related activities (e.g., sporting events or clubs) that require travel between campuses during the school day.
