    By signing, you agree to:

    Arrive on time (late arrivals may be canceled)

    Come prepped (clean skin, no lotions, deodorant, or makeup)

    Follow all pre-care and aftercare instructions provided

    Be comfortable being photographed and/or recorded

    Allow content to be posted publicly

    Not edit or alter photos/videos without approval

    Tag and credit the artist when posting

    By signing this form, I grant permission to Vixyn Glow to:

  • ♡Take photos/videos of me before, during, and after my spray tan. ♡Use this content for marketing, social media, website, ads, and promotions. ♡Use my likeness without compensation or royalties

  • 5. RESULTS DISCLAIMER

    Spray tan results vary based on skin type, prep, and aftercare

    Uneven fading, transfer, or tone variation is possible

    This is a cosmetic service, not permanent

    6. LIABILITY WAIVER

    I understand that:

    I am voluntarily receiving a spray tan service

    I have disclosed any skin sensitivities, allergies, or conditions

    I release the artist and business from any liability related to reactions, staining, or dissatisfaction

    7. CANCELLATION POLICY

    Models must give at least 24-hour notice to cancel/reschedule

    No-shows or last-minute cancellations may result in being removed from future model opportunities

    8. AFTERCARE AGREEMENT

    I agree to follow all aftercare instructions including:

    Avoid water, sweating, and tight clothing for the recommended time

    Moisturize daily after first rinse

    Avoid harsh exfoliants until tan fades
