MSYO Medical Release Form
I, the undersigned, as the parent or legal guardian of the above-named minor, hereby authorize the staff and representatives of the Modesto Symphony Orchestra Association (MSOA) to act on my behalf in any emergency requiring medical attention for my child.

I grant permission to transport my child to the nearest hospital or appropriate medical facility, and for emergency medical treatment to be administered if necessary, including anesthetic or surgical procedures, as deemed necessary by a licensed medical provider.

I understand that reasonable precautions will be taken to ensure the safety and well-being of all participants. I hereby release and hold harmless the Modesto Symphony Orchestra Association (MSOA), its staff, volunteers, and agents from any liability for injuries, loss, or damage to personal property during participation in this program or event, except in the case of gross negligence or willful misconduct.
