1. Graduating from high school and choose not to apply for the Jack Summerall Scholarship, we would like for you to submit your information for recognition.

2. Graduating from college receiving a Certificate, Associate, Bachelor, Master or Doctorate degree; please submit your information as well for recognition.

3. Forms must be postmarked or emailed by July 15, 2026. We cannot guarantee your infomation will be included in the JSFG Souvenir Booklet if postmarked or emailed after July 15, 2026.
