I, the undersigned parent/legal guardian (or participant if 18 or older), acknowledge and agree to the following terms on behalf of myself and/or my minor child as a condition of participation in any and all activities associated with Blaze Cheer and Tumble during the 2026–2027 season.
1. MEDICAL AUTHORIZATION & RESPONSIBILITY
I understand that the staff of Blaze Cheer and Tumble are not physicians or medical professionals. In the event of injury or illness, I authorize Blaze Cheer and Tumble, its coaches, staff, and representatives to administer basic first aid and/or seek emergency medical treatment, including calling emergency medical services. I agree that all medical expenses incurred are my sole responsibility, and I affirm that I carry valid health insurance coverage for the participant.
2. ASSUMPTION OF RISK
I fully understand and acknowledge that participation in cheerleading, tumbling, gymnastics, stunting, travel, competitions, practices, and all related activities involves inherent and significant risks, including but not limited to:
Sprains, strains, and soft tissue injuries
Broken bones and concussions
Catastrophic injuries including paralysis or death
These risks may occur at the gym, during team events, while traveling locally or nationally, at competitions, or in any location associated with Blaze Cheer and Tumble activities.
I voluntarily assume ALL risks, both known and unknown, even those arising from negligence.
3. RELEASE OF LIABILITY (INCLUDING NEGLIGENCE)
To the fullest extent permitted under the laws of the State of Texas, I hereby release, waive, discharge, and covenant not to sue:
Blaze Cheer and Tumble, including its owners, directors, officers, coaches, employees, volunteers, agents, contractors, representatives, and affiliated staff
from any and all liability, claims, demands, actions, or causes of action arising out of or related to:
Injuries sustained at the gym or on the premises
Injuries sustained off-site, during team activities, events, or appearances
Injuries sustained during travel (local or national), including transportation, lodging, and competitions
Illness, disease, or medical conditions arising from participation
Property damage or loss
This release specifically includes claims based on negligence, whether active or passive.
4. NO LIABILITY FOR INJURIES (ON OR OFF PREMISES)
I expressly acknowledge that Blaze Cheer and Tumble is not responsible or liable for any injury, illness, damage, or loss that occurs:
Inside the facility
Outside the facility (including parking lots and surrounding areas)
While traveling with or for the team
At competitions, events, or third-party venues
I accept full responsibility for any such occurrences.
5. TRAVEL & COMPETITION LIABILITY WAIVER
I understand that participation may involve regional and national travel. I agree that Blaze Cheer and Tumble and its representatives are not liable for any injuries, accidents, delays, losses, or incidents that occur during:
Air travel, ground transportation, or lodging
Competition venues or event spaces
Any travel-related activity connected to the team
I assume full responsibility for all risks associated with travel.
6. COMMUNICABLE DISEASE / HEALTH RISKS
I acknowledge the risk of exposure to communicable diseases, including but not limited to COVID-19. I voluntarily assume all risks related to illness and agree that Blaze Cheer and Tumble is not liable for exposure, infection, or spread of any illness.
7. FINANCIAL RESPONSIBILITY FOR INJURY
I agree to be fully and solely responsible for any and all medical bills, treatment costs, therapy, or related expenses resulting from participation, regardless of where or how the injury occurs.
8. MEDIA RELEASE
I grant Blaze Cheer and Tumble permission to photograph, video record, and use the likeness, voice, and image of myself and/or my child for promotional, marketing, and social media purposes without compensation.
9. SEVERABILITY & GOVERNING LAW
If any portion of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
This agreement shall be governed by and interpreted in accordance with the laws of the State of Texas.
10. ACKNOWLEDGMENT OF UNDERSTANDING
I certify that I have read this agreement in full, understand its contents, and voluntarily agree to all terms. I understand that I am giving up substantial legal rights, including the right to sue.
