Finasteride-Specific Informed Consent: Please read the following carefully before signing. This consent is in addition to the general FollicaRx Advanced consent and addresses the specific risks of the finasteride component of your prescribed formula.I understand that finasteride is a 5-alpha reductase inhibitor that is FDA-approved in oral form for the treatment of male pattern hair loss and benign prostatic hyperplasia. The compounded topical finasteride included in FollicaRx is prepared by a licensed compounding pharmacy, and its use in this topical form for hair loss is considered off-label. I understand that compounded topical formulations are not FDA-approved as products and have not undergone the same large-scale safety and efficacy testing as FDA-approved drugs.I understand that while topical application is intended to reduce systemic absorption compared to oral finasteride, measurable amounts of finasteride can still enter the bloodstream and reduce circulating dihydrotestosterone (DHT). As a result, side effects associated with oral finasteride remain possible, including but not limited to: decreased sexual desire, erectile dysfunction, problems with ejaculation, decreased semen volume, breast tenderness or enlargement (gynecomastia), allergic reactions, skin irritation at the application site, mood changes including depression and anxiety, and, in rare cases, suicidal thoughts. There have also been post-marketing reports of sexual, neurological, and mood-related symptoms persisting after the medication is stopped — sometimes referred to as "post-finasteride syndrome" (PFS) — although the frequency and underlying mechanism of persistent symptoms are not fully established. For most patients, side effects resolve after discontinuation, but I understand that resolution is not guaranteed.I understand that finasteride may decrease serum prostate-specific antigen (PSA) levels by approximately 50%, which can affect prostate cancer screening, and that I should tell any provider ordering a PSA test that I am using this medication. I understand that finasteride has been associated, in some studies, with a small increase in the risk of high-grade prostate cancer detection. I understand that this medication is not approved or studied for use in patients under 18 years of age, in women, or in pregnant or breastfeeding individuals.I understand that finasteride can cause serious birth defects to a developing male fetus, and that women who are pregnant or who may become pregnant must not handle this medication or come into contact with my application site while the medication is wet or absorbing. I agree to apply this medication only to the area instructed, to keep the application site covered or fully washed and dry before any skin-to-skin contact with a pregnant person, anyone who may become pregnant, anyone who is breastfeeding, or anyone under 18, and to store the medication securely out of reach of children and pets.I agree not to donate blood, plasma, sperm, or other tissue while using this medication and for at least one month after my last dose, because finasteride can be present in donated material and could expose a pregnant recipient. I will inform any future healthcare provider, including any provider ordering a PSA test, that I am or have been using finasteride.
