I understand that written notice of warranty concerns must be submitted within 2 years of my closing date, that certain items are excluded from coverage (including boilers, hot water heaters, oil tanks, appliances, and items listed in the General Exclusions section of the Limited Warranty Certificate), and that submitting this form constitutes written notice of the concern described above.
This form is for non-emergency warranty concerns. If you are experiencing a safety emergency such as fire, gas leak, or injury, call 911 immediately.
