I give permission for my child/ren to attend the excursion to Bounce and to participate in all supervised activities associated with this event. I acknowledge that while all reasonable care will be taken by organisers, participation in trampoline and recreational activities carries a level of inherent risk. I understand that my child will be transported to and from the venue in a vehicle driven by a fully licensed driver. I authorise C3 Youth to seek medical treatment for my child in the event of an emergency and accept responsibility for any associated costs. I confirm that my child is physically able to participate and that any relevant medical conditions or concerns have been disclosed.
