AGREEMENT, UNDERSTANDING, AND RELEASE
Please enroll my child in this FCS Camp.
The undersigned as parent/guardian understands, agrees, and acknowledges as follows:
1. I assert that my child is physically fit and able to participate in this camp.
2. In the event that the enrolled camper requires emergency medical treatment or hospital admission, and circumstances do not allow timely contact of the undersigned by the school official, school officials are hereby authorized to take actions they deem necessary for the emergency medical treatment of the camper.
3. FCS, FCS Employees, FCS Volunteers, and FCS Administration are fully released from any and all liability for loss, damage, illness, or injury suffered by my child. I indemnify the school, its employees, its volunteers, and its administrators and hold them harmless against claims or suits made or brought by anyone because of injury, illness, or loss.
